Table 2: Category 1 Rules

Specification v3.4.0, section III.C

The approved combinations provided in Table 2 are the baseline matching rules eligible for implementation . Additional detail on model construction and threshold selection are provided in a later section.

As of v3.4.0, Table 2 is divided into Category 1 (Direct Match Rules) — 30 rules evaluated in a single step, exactly as in v3.3.0 — and Category 2 (Household + Individual Rules) — 8 rules evaluated in two steps.

Table 2 Approved matching combinations and conservative collision estimates. Fields with a * may be fuzzy.

Except where this framework expressly provides a deterministic contraindication, disagreement in additional data elements present in the query or source record, but not included in the qualifying Table 2 combination, does not by itself negate the match. In such cases, the more likely implication is that one or more non-qualifying data elements are inaccurate, stale, or incomplete, not that the otherwise qualifying match is incorrect. Organizations MAY flag such queries for review, audit, or data-quality follow-up. This framework further mitigates risk of an incorrect match by requiring that a match is satisfied only when it produces a unique candidate subject at the source system.

Notes: First name is the twin and multiple-birth guardrail for rules that rely on household-shared demographic evidence. Where a fuzzy value is shown with "*". Email, SSN last 4, ITIN last 4, MBI, Legal ID, ZIP, and namespace bound unique identifiers must match exactly. MBI means CMS-issued MBI only; generic insurance subscriber or member identifiers are not interchangeable with MBI under Table 2.

When Date of Birth is shown with “*” that means we allow a ±1-day tolerance.

Notes: Member ID means the full individual-level identifier inclusive of any dependent suffix; the bare subscriber base value without a dependent suffix SHALL NOT be used under Member ID rules. Subscriber ID means the policyholder-level base value; some rules require at least one individually discriminating demographic field (first name, last name, or date of birth) to mitigate recyclability risk. For all rules in this group, the Payer ID establishes the namespace and matching SHALL be performed only within that namespace. Email, phone, date of birth, subscriber ID, and member ID must match exactly after normalization. Fuzzy matching is limited to name fields as defined in Table 3 and §V.E.
