Per-Field Collision Probabilities

Specification v3.4.0, section IV.C

For a single field with counts ni across N distinct individuals, the exact non-match agreement probability is:

ufield = Σ[ni(ni - 1)] / [N(N - 1)]

For large populations, this may be approximated as ufield ≈ Σ(fi^2)
where fi is the relative frequency of value i. When a field is approximately uniform, a simpler approximation is

ufield ≈ 1 / Ndistinct

In practice, exact population frequency distributions are not available for every field, and field distributions vary across patient populations, regions, and source systems. Accordingly, the values in Table 3 should be treated as conservative single-point assumptions based on reasonable expectations about cardinality, skew, missingness, and data quality. They are intended for threshold setting, comparative scoring, and rule classification, not for precise actuarial measurement.

Fuzzy matching is used only to account for limited data-entry variation that remains after deterministic normalization. It is not applied to structured fields such as DOB, ZIP, MBI, Legal ID, or namespace bound unique identifiers, and it is not applied to any compared string shorter than 5 characters after normalization. Fuzzy comparison may be used only for first name, last name, and street line, and only under constrained rules intended to capture minor typographic or adjacent transposition errors. Unless Table 2 expressly states otherwise, no more than one field in a qualifying combination may be satisfied by fuzzy comparison.

Table 3. Conservative per-field u-probabilities used to classify Table 2 combinations

FieldConservative u, exactConservative u, fuzzyBasis / assumption
First Name0.020.03Common-name concentration; deterministic nickname handling first; fuzzy limited to a single minor edit or adjacent transposition and only when the normalized value is at least 5 characters.
Middle Name0.01Not usedThis field has been dismissed due to observed data quality issues and low selectivity
Last Name0.0050.01Heavy-tailed surname distribution; no phonetics; fuzzy limited to a single minor edit or adjacent transposition and only when the normalized value is at least 5 characters.
Suffix0.2Not usedThis field has been dismissed due to observed data quality issues and low selectivity
Date of Birth (full)0.0001Not usedExact comparison only.
Year of Birth0.015Not usedThis field has been dismissed due to observed low adoption and low selectivity
ZIP Code (5-digit)0.0003Not usedConservative geographic concentration assumption; exact comparison only.
City0.01Not usedExact comparison only.
State0.06Not usedExact comparison only.
Street line0.000030.00006Street line and ZIP standardized; ZIP must remain exact. Fuzzy, when allowed, applies only to the street line.
Phone Number0.000001Not usedNormalized to E.164
Email Address0.000001Not usedExact comparison only after deterministic normalization.
SSN Last 4 Digits0.0001Not usedExact comparison only.
ITIN Last 4 Digits0.0001Not usedExact comparison only.
Legal ID (full exact value within issuing-authority namespace)0.000001Not usedConservative floor for exact DL/passport or equivalent legal identifier; exact comparison only within namespace.
MBI (CMS-issued Medicare Beneficiary Identifier)0.000001Not usedCMS-issued MBI only; generic insurance identifiers are excluded unless separately validated.
Namespace bound unique identifiers (e.g. EMPI, FHIR Patient Identifier, CSP UUID, etc.)≈0Not usedExact comparison only; treated as effectively zero collision for under namespace-bound exact matching.
Insurance Member ID (full individual-level value within payer namespace, i.e., including any dependent suffix)0.000001Not usedConservative floor for exact individual member identifier issued by a single payer; exact comparison only within payer namespace. Family-plan dependent suffixes must be included; a bare subscriber base value without dependent suffix SHALL NOT be treated as a Member ID under this row.
Insurance Subscriber ID (policyholder-level base value within payer namespace)0.0001Not usedConservative estimate reflecting documented family-plan sharing: approximately 40% of commercially insured individuals are dependents sharing a base subscriber ID with one or more household members. This degrades the effective per-individual selectivity to roughly the level of SSN last 4 digits. Exact comparison only within payer namespace.
Relationship Linkage — clinical source0.01
Relationship Linkage — self-reported/intake0.05

Where no reliable empirical distribution is available, the assigned u-value should be understood as a conservative order-of-magnitude assumption or engineering floor rather than a directly measured quantity. For namespace bound unique identifiers, "≈0" indicates effectively zero collision for threshold design under exact namespace-bound comparison, not a claim of literal impossibility.

The following simplifying assumptions are reasonable for threshold-setting purposes:

  1. City and state should generally be treated as a single geographic signal when both are present.
  2. ZIP code already captures the state and much of the city, so the state adds little incremental discrimination when ZIP is available.
  3. Address line 1 and ZIP should not automatically receive full independent credit, because they are related geographic fields.
  4. Phone number and email address should be treated as strong but imperfect identifiers, since either may be shared, stale, reassigned, or missing.
